WHAT IS YOUR NEW PATIENT PROCEDURE?

How do I get started?​​

You can start by calling our office at (860) 852-1566 option 3 or email us at LweberaprnLLC@hotmail.com

We will reach out to you to make an initial inquiry about your contact and insurance information and ask you for a brief description of your treatment needs and concerns. It is important to get a brief description of your needs so we can determine if outpatient treatment is the appropriate level or type of care for you or your child.

If this is the appropriate care for you, we will set up an appointment, send you all office policies through our EMR and collect all office forms that are necessary. All new client forms are due before the first appointment.

HOW CAN I FIND OUT WHAT MY INSURANCE COVERAGE IS? WILL I HAVE A COPAY? WHEN ARE PAYMENTS DUE?

We participate with numerous insurance companies, but not all.  While you will be provided with the best information available, it is your responsibility to check with your insurance company prior to each visit to verify coverage and benefits.

 

Our Insurance page provides information on the insurances we are in network with and direct links to each insurance company so you can check your benefits online. You also have the option to call the Member Services number typically located at the back of your insurance card.

Insurance co-payments- You are required to pay any copay and/or deductible at the time of the visit. Payment may be made with cash, check, or accepted credit cards (Visa, MasterCard). A $10 clerical fee may be added if we have to bill you.

Checks should be made out to Lee Weber APRN LLC.

Private payment- Full payment is due on the day of services; this allows us to avoid clerical costs for paperwork, billing and postage. Fees vary per provider/clinician and are available upon request. All clients are required to keep a credit card on file as a default payment method. Please inform us if you require a claim form to submit to your insurance carrier for reimbursement.
